Welcome to Nourished with Nutrition! The website, www.nourishedwithnutrition.com.au is owned and operated by Rachel Kane operating under the Australian company name Nourished with Nutrition Pty Ltd. Please read the following Terms and Conditions together with the Privacy Policy when using this website, making a booking or a purchase.

When you visit this website, use our services or purchase our products, you agree that you are over the age of 18 and willing to
be bound by these Terms and Conditions, give your consent and are aware of our privacy policy. If you continue to use this
website, and purchase any of its products and services you are agreeing to the following Terms & Conditions. Your privacy is
important and the information collected from you is to provide nutrition services to you, please ensure you read the Privacy
Policy here. Consent for the course of your treatment with Nourished with Nutrition is obtained by accepting these full Terms
& Conditions. You may withdraw consent at any time by emailing rachel@nourishedwithnutrition.com.au. If you do not agree
with these Terms & Conditions, the Privacy Policy and/or do not consent to treatment, please do not use the website or make
any further bookings.

Treatment

Nutritional treatment takes commitment from both you as the client, and myself as the practitioner. To get the most out of
your nutrition consultations please start by completing the intake form and any other recommended forms (which may
include a diet diary, mood journals etc) as thoroughly as possible. I work with clients to make incremental and lasting changes
to their diet and lifestyle, from wherever you are at on your health journey. As a nutritionist I will not diagnose any conditions
but work to identify and rectify underlying causes of your presenting signs & symptoms. Treatments may include a range of diet and lifestyle modifications, as well as nutritional medicine where evidence suggests its likely to produce a therapeutic benefit.

Please understand results are not guaranteed and you may refuse treatment at any time without prejudice from me as your
practitioner. If you feel our partnership is not working, I can recommend another practitioner for you to work with. If any
changes occur to your health status (eg. pregnancy, new test results or your Doctor prescribes you a new medication) please
book a follow up consult so we can discuss in detail. It is important you do not cease existing medical treatment or self-medicate without consulting your doctor. The treatment plans and supplement prescriptions are individually tailored to meet
your health goals only, so please do not share with others to ensure safety and efficacy. Please ensure you have notified us of
any allergies or intolerances prior to your initial consultation. If you experience an unexpected allergic reaction to a product
prescribed, please notify as soon as possible. Any emergencies must be directed to 000 immediately.

Scope of Practice & Code of Ethics

As a member of the Australian Natural Therapists Association (ANTA), Nourished with Nutrition agrees to treat each client
within its scope of practice and abide by their code of Professional Ethics as set out by ANTA. To read more click here for Scope and Standards of Practice and here for the Code of Ethics.

Payment

For consultations, testing and product sales made online, full payment is required at time of booking/purchase. Nourished with Nutrition uses the square to process online payments. If you would prefer to make a direct deposit please email rachel@nourishedwithnutrition.com.au.

Cancellation, Rescheduling & Refund Policy

A minimum 24 hours-notice via email or link in your booking confirmation is required to reschedule your consultation. Please note that cancelling via email with less that 24-hours notice or not showing up for your appointment will result in $50 cancellation fee being charged as short notice cancellations do not allow time for other clients to book. To reschedule email rachel@nourishedwithnutrition.com.au.
Note: If we experience any unforeseen technical difficulties and cannot continue our consultation, we can continue the consultation via telephone, or reschedule to another suitable time.

Health Fund Rebates

A rebate may be available from your private health fund for nutrition consultations depending on your policy and provider. Please contact your individual health fund for more information.

Professional Insurance Cover & Limitation of liability

My insurance covers Australian in-person and online (telehealth) Nutrition consultations.
Liability is limited to replacing the goods or providing the services again, if you are not happy with a service please contact
rachel@nourishedwithnutrition.com.au within 7 days of your appointment.

Email Policy

Email Policy Contact can be made through rachel@nourishedwithnutrition.com.au email or the Patient Portal, please allow up to 48 hours for a reply. I hope you understand as a duty of care I cannot give treatment advice over email. In cases of emergency please contact the appropriate services such as Emergency (000), Lifeline (131 411) or your local hospitals emergency department.
